To obtain a birth or death certificate, you must do the following:

1. Complete the Application for a Certified Copy of Birth/Death Certificate, sign it, and submit it to the Clerk of Court's office in person for processing.
2. A copy of this form may be obtained at the Clerk of Court's office or by using the link below.
3. Present a current ID.
4. Make payments with cash, credit card, or debit card.
5. If your relationship to the person on the certificate is not listed on the application, you must also complete the Louisiana Vital Registry Release Authorization Form (link provided below).

Fees

• Birth certificate: $34.00
• Birth certificate and birth card: $48.00 (a birth card can only be purchased along with a birth certificate)
• Death certificate: $26.00 (death certificates are only available dating back to July 7, 2012)
• Mailing: an additional $5.00 for regular mail or $10.00 for certified mail
